Tell us about a child placement

You can use this form to tell us about the placement of a looked after child in Stockport.

Before you start

You'll need:

• details of the child or young person
• details of the child or young person's social worker
• the child's address before they were a looked after child, their previous placement address and their current placement address

You'll also need to upload any supporting documents including any risk assessments and action plans relating to the child and a copy of the child's Education Health Care Plan (EHCP) if they have one.
